1. Get your parish forms
Holy Spirit publishes a Faith Formation Volunteer Eligibility form. Check with the parish office for the current paperwork and where to return it.

The Diocese of Sacramento directs parish volunteers working with children to Safe Haven training through CMG Connect. Use the parish registration portal below.

Get the Request for Live Scan Service form from the parish office before arranging fingerprinting. The diocese’s instructions explain background checks for volunteers with regular contact with children.
